Preceded in death by her parents, Donald & Gladys McGrath; former husband, John Hasenstab; her son, Steven Hasenstab.
Loving mother of John M. (Mary Ajack) Hasenstab, Susan Hasenstab, JoAnn (Ben) Walldren and Steven (Marcy) Hasenstab. Proud grandma of Matthew (Jeanne) Hasenstab, Cara (Mark) Cobalt, Sara Hasenstab, Jemme Harper, Elizabeth (Michael) Koeppler and Michael Bednarek. Great-grandma of Dexter Walsh, Elliot Hasenstab and Lily Koeppler.
Phyllis's family extends their thanks and gratitude to the wonderful nurses and staff at Zilber Family Hospice, Southpointe Healthcare Center, and Aurora In-Home Hospice Care.
Private burial will be held.
