Tracey Ann Polzin (Nee Helm) lost her courageous battle with cancer at home with her family on May 9th, 2017 at age 55.  Preceded in death by son Jesse D., mother JoAnn DeCleene, father Jack Helm and brother Steve Helm.  Survived by caring husband Jeff Polzin, who lovingly and tenderly cared for her until the Lord called her home.  Also survived by loving daughter Stevie Cynthia Guhr (Dan) and son David Helm.  Further survived by two adored grandsons Levi and Kash Guhr, brother Randy Helm, sister Kim Helm, cousin Jeff Wilson, Auntie Sally and many special nieces and friends. Visitation will be held on Tuesday, May 16th from 4pm to 6pm at the funeral home. Service at 6pm.In lieu of flowers, memorials to the family are greatly appreciated.
